GOLD LEVEL SPORTS & ENTERTAINMENT, CHRIS BOUCHER’S BACK TO SCHOOL CAMP EVENT ATTENDEE WAIVER AND RELEASE OF LIABILITY FORM

 

Gold Level Sports & Entertainment (GLSE) continues to monitor policy and procedures during the COVID-19 pandemic. The safety and well-being of our campers, staff, and parents at our events is our top priority. We are collaborating with provincial agencies, and the venue to implement best practices aimed at managing the spread of the novel and complex virus.

 

GLSE has put in place preventative measures to reduce the spread of the COVID-19 virus, however, are under no circumstances, able to guarantee that our participants will not become infected with the virus whilst at the event venue and in surrounding areas.

 

Please review agreement below. Additionally, your signature will be required as a confirmation of your intent and commitment to abide by policies put in place to protect visitors/guests and staff.

 

Agreement:

☐ I declare that I, nor my child is experiencing or exhibiting any COVID-19 related symptoms as outlined by the provincial Center for Disease Control and Prevention (CDC) such as fever, dry cough, or shortness of breath.

☐ I declare that I, nor my child has traveled internationally or to a highly impacted COVID-19 area within Canada during the last (14) days.

☐ I am not aware, nor do I believe my child has been recently exposed to a person or being with a positive and confirmed case of COVID-19.

By signing this event attendee waiver and release of liability form, I acknowledge the contagious and evolving nature of COVID-19 and voluntarily assume the risk that my child may be exposed to the virus and become affected as a result. I understand the risk of becoming infected which may impact event staff and event attendee’s families or personal contacts.

 

I voluntarily agree on behalf of my child to assume all of the foregoing risks and accept sole responsibility for any expense, liability, illness, injury, disability or death related to contracting the virus while attending an event hosted by GLSE. I hereby release, covenant not to sue, discharge, and hold harmless GLSE and affiliates from all claims of any kind arising out of COVID-19. I understand and agree that release of liability includes any claims towards GLSE, staff, board, partners, personnel, and representatives whether a COVID-19 infection occurrence exists before, during, or after attendance and participation at an GLSE event.

 

 

This event attendee waiver and release of liability form shall be governed by the province in which the event is being held without regard to its choice of law principals. I agree not to commence or prosecute any action in connection herewith other than in the province in which the event takes place. I agree to be bound by all terms of this event attendee waiver and release of liability form, as indicated by my signature below. Please print waiver and present it to our staff upon arrival at the facility
